"What is the easiest way to find the forms I need?"

There are several different ways to access the forms you need, depending on the reason you need these forms.

Permit Application:  If you are seeking permission from TTB to operate in the beer, wine, distilled spirits, industrial alcohol, tobacco or firearms/ammunition industries, your best bet is the Application Packets page. Simply select the industry you are interested in entering and you’ll be presented with a page containing the forms and instructions you need.

Commodity Home Pages:  If you are already a member of a regulated industry, you can find all forms associated with operating your business via the appropriate commodity home page.  In the top navigation bar just beneath the banner above, select Beer, Wine, Distilled Spirits, Other Alcohol, Tobacco or Firearms & Ammunition as appropriate.  On the left hand side of the page you’ll find a Forms and Publications link which leads to a web page containing all pertinent forms for that commodity.

Main Forms Page:  Alternatively, you can access the TTB.gov Forms page. This web page provides access to forms in a variety of ways.  In addition to a web page displaying all TTB forms, this page provides a list of frequently requested forms, forms by commodity, resources to assist you in correctly completing forms and options for commenting to TTB on any form.

Search:  Another way to locate a specific form is to utilize the TTB.gov search engine. If you know the form number, you may search it and a link to the form will be presented as the first link on the results page. Alternatively, you can enter key words that might be found in the title of the form.  The form in question should appear on the first page of search results.
