Amended Application for Entity Information

Note: This page contains instructions and screenshots so you can see what to expect when using Permits Online, our online method of applying for a TTB permit, registration, or notice.

File this application if you need to:

  • Change controlling ownership
  • Change any partner or general partner
  • Change any stockholder or members holding ownership of 10 percent or more
  • Change corporate officer, directors, or any titled positions
  • Add, revoke, or change signing authority
  • Add, revoke, or change power of attorney
  • Change the legal business name

Note: You may submit multiple amendments in one application.
