New Application Wizard, Original Entity, and Personnel Questionnaire

New Application Wizard

The New Applications Wizard guides you by asking a series of questions, and depending on your answers creates an application package, which is a list of the applications you're required to submit.

Everyone who applies for a new TTB permit, registration, or notice must complete the questions in the New Application Wizard.

Application for Original Entity

When applying to start a new TTB-regulated business, your application package will contain both an application for original entity and a new commodity operations application (one for each permit, registration, or notice you need).

Personnel Questionnaire

The Personnel Questionnaire is how TTB gathers background information about owners, officers, and others associated with TTB-regulated businesses.
